Creatine safety questions often arrive as one bundle: Will it cause bloating? Is a higher number on the scale body fat? Does it increase dehydration, cramping or kidney problems? The strongest general conclusion applies to healthy people studied under defined supplementation protocols. In a double-blind trial of 59 male football players, participants took either two 5-gram serves, one 10-gram serve or placebo each day for 28 days. That study supports a narrow practical conclusion: a large single serve may be harder to tolerate than the same daily amount divided into smaller serves. Is water stored with muscle creatine the same as digestive bloating?

Existing kidney or liver conditions and medication use

A 2025 review reported that controlled studies did not show adverse renal effects in healthy individuals, while advising caution where kidney conditions already exist and identifying evidence gaps for pregnancy. A 2025 systematic review and meta-analysis found a modest increase in serum creatinine without a significant change in glomerular filtration rate across the included studies. Because creatine supplementation can affect serum creatinine without necessarily indicating reduced kidney filtration, people taking creatine should inform their healthcare professional so this can be considered when interpreting test results. 4,5,7
